Certification of Completion Insurance form

Upon completion of second driving session, students will be given a certificate of completion to be turned in to their insurance provider for safe drivers discount.  This form proves that the student listed on the form has successfully taken and passed a SC Drivers Education class.  

Replacement copies can be requested by email (lakelandsdrivingacademy@gmail.com) or by calling 864-323-2226.  There is a $20 charge for replacement copies.
